There's no request, it's an automatic process. There's no additional cost to the CA-4 it's just there to calculate the time you've spent in all four countries.

The CA-4 is the approving stamp to enter the four countries so there's no extra work involved. Each time you enter one of the four countries they will calculate and inform you how many valid days, out of the 90 allotted, you have left. This doesn't mean the entry/exit fees are the same though...
